This film continues directly from the ending of the first Underworld film. The two main characters Selene a vampire warrior played by Kate Beckinsale, and Michael a hybrid vampire/werewolf played by Scott Speedman. While this is the second movie of the franchise, it is chronologically the third installment. The film was critically not successful, but it was a very profitable film overall. The original cut of the film was over two hours long and lacked the CGI visual effects, and proper sound mixing. It was also a much more graphic movie, with nudity and a large body count of 61, but was cut down to avoid an NC-17 rating according to the director Len Wiseman. The filming schedule was short only being 71 days. Underworld movies are known for their slow-motion effects, and use of guns in many scenes. Instead of the traditional concept of vampires, and werewolves being supernatural in nature in the Underworld films it is presented as a disease.

This is the second in a franchise of five films starring Kate Beckinsale in the lead role, with Scott Speedman. It was nominated for multiple Fangoria Chainsaw Awards and won an MTV Movie Award for the Best Hero for Kate Beckinsale as well as a Scream Award for Best Scream Queen.
