The film follows a group of friends in a small rural town in Massachusetts, who after summoning the Slender Man, find that their friend Katie has disappeared, presumably taken by The Slender Man. The remaining friends attempt to contact the Slender Man again to get Katie back, and as the film progresses, they and their immediate family and acquaintances encounter the Slender Man and cope with the resulting fear and insanity the evil being caused.

The Slender Man concept is credited to an internet horror story known as a 'creepy pasta', written by Eric Knudsen, who goes by the online name Victor Surge. Slender Man is a tall, elongated figure, often described and portrayed as having no facial features, and many arms branching out like that of a tree. As the character has grown in popularity online, the lore and mythos of his have been greatly expanded, with tons of fan art made of him as well. The concept of the Slender Man is heavily influenced by the works of author HP Lovecraft, who specialized in dark horror stories of otherworldly creatures, and psychological torment for the characters who encountered them. In many of Lovecraft's works, and in Slender Man mythos, individuals would often go insane by simply looking at a creature, as it was described as so unfathomably horrific, the human mind cannot even comprehend what they have seen. The Slender man has long since been given more concrete appearance and definition than many of Lovecraft's creations, but this concept of psychological horror and insanity is very much alive in the Slender Man mythos and its many adaptations.
