Six friends take a trip to Mexico. While there, Olivia meets a man named Carter who invites her and her friends for drinks at an old mission's ruins. Carter suggests a game of Truth or Dare and the others agree. At the end of the game, Carter tells them that they have taken his place in a supernatural game of Truth or Dare and that the game will follow them through their lives. After returning from Mexico, Olivia tries to convince her friends that the game was real. One friend is forced to break another's hand while another has to reveal that his father is homosexual. One is dared to walk on the roof; she falls but is saved by the other friends. The friends meet another person who has also played this version of the game and discovered that they were responsible for the game becoming part of reality. Each of the remaining friends is forced to take their turns, with deadly consequences for some of them. Eventually, Olivia learns the secret of how to prolong the game so that they do not have to take another turn for a very long time.

Truth or Dare is a party game requiring at least two players. Each person takes a turn, choosing either 'truth' or 'dare.' If the player chooses 'truth,' they must truthfully answer a question given to them by the group. If the player chooses 'dare,' s/he must complete a daring act suggested by the group. Players are not allowed to switch to the other choice or quit the game once the question or dare has been announced.

It stars Tyler Posey as Lucas Moreno, Lucy Hale as Olivia Barron, Violett Beane as Markie Cameron, Hayden Szeto as Brad Chang, and Landon Liboiron as Carter. The film was directed by Jeff Wadlow, who also co-wrote the screenplay, and is produced by Jason Blum.
